Volunteer Requirements

As a first time volunteer in the Tunkhannock Area School District you will be required to submit the following: 

  1. Volunteer Data Sheet           
  2. TB Test  
  3. Act 34 (PA State Police Criminal Check Record) 
  4. Act 151 (PA Child Abuse History Certificate)  
  5. Disclosure Statement for Volunteers – which is a statement swearing or affirming the applicant has not been disqualified from service by reason of conviction of designated criminal offenses or being listed as the perpetrator in a founded report of child abuse. (For volunteers who have been a resident of PA for the entirety of the previous ten (10) year period)

              Or  

If the volunteer has not been a resident of PA during the entirety of the previous ten (10) year period, the volunteer must also submit a Federal Criminal History Report.

6. Act 24 Arrest/Conviction Report and Certification Form

7.Acknowledgement of Volunteer Obligations & Confidentiality Agreement

 

If you continue to volunteer in subsequent years, clearances that are less than (60) sixty months old and without a break in service, you will only need to submit the Volunteer Data Sheet, Act 24 and Acknowledgement of Volunteer Obligations & Confidentiality Agreement. You must be Board approved every year.

 

Completed packets should be submitted to the office of the building where you wish to volunteer or to the Athletic Director. If you have students in multiple buildings, only one packet needs to be submitted. Once Board approved, your name will be listed on the approved volunteer list which is distributed to all buildings and the Athletic Director.
